Applicant does hereby authorize the investigation of all statements contained in the application and does certify that all statements are true and that any misrepresentation or omission is cause for the Management and/or the Owner to reject the application and/or terminate the lease. Applicant does hereby authorize acquiring a credit report, criminal back ground check, employment and landlord verification for the purpose of determining applicant’s worthiness. Agent has continuing right to review my credit, criminal & employment.

Please fill this out and have every person on the lease sign and date. When you come in to sign your lease renewal, bring this completed form, current proof of income, and current proof of insurance.
